Using the form
Drop this shortcode anywhere — a page, a post, a widget area, or inside any popup/modal plugin you already use:
To have the plugin render its own popup (a button that opens the form in a modal, no other plugin needed):
To lock a form to one campus (hides the Campus dropdown, e.g. on a campus-specific landing page):
Everything else — dropdown population, cascading Program/Class options, validation, submission, double-submit protection, honeypot spam-guarding — is handled automatically. Nothing to configure per form.
Elementor (optional, advanced)
If you’d rather build the form yourself inside Elementor Pro with full control over layout, a “Send to GlowSIMS” Actions After Submit option is also available — see the plugin README for field-mapping instructions. Most sites won’t need this; the shortcode above covers it.
